Before you apply

  • File your most recent tax return (and your partner's). Eligibility is based on it.
  • Confirm you don't have access to private dental insurance.
  • Have your Social Insurance Number and address details ready.

Steps

  1. Apply via Service Canada (online or phone).
  2. Income check: the CRA confirms your adjusted family net income.
  3. Enrolment: if approved, the government sends an enrolment letter with your member ID and coverage start date; Sun Life administers the plan and mails your welcome package (member card + plan details, and your start date again). The package can take up to three months — you don't need to wait for it to get care.
  4. Get care: visit a participating provider on or after your start date.
